The problem

How we work
A useful recommendation needs a reason, a responsible person and evidence of what happened next.
A change in performance, a training gap or unused capacity should lead to a clear next step, with someone responsible for following it through.
Calculations, eligibility and expiry use traceable rules. AI can explain or suggest; accountable people assess capability and make clinical decisions.
Keep source dates and limitations visible. Distinguish an expected benefit from a result that has actually been observed.
Learning and attendance are recorded separately from observed practice, assessment and current sign-off. Each has a different role in demonstrating capability.
